You may DROP OFF, EMAIL, or MAIL completed forms along with a $35.00 non-refundable Registration Fee (per student) payable to Plumb Performing Arts Center, 9153 E. Bell Road, Scottsdale, Arizona 85260. If you choose to email your forms, you may call the office with a credit card number.
